Student Enrollment Information
All students must present an immunization record (Kansas Certification of Immunization) signed by physician’s office, Health Department.
The following immunizations are required prior to school entry:
- DIPTHERIA, TETANUS, PERTUSIS (DTP).
- MEASLES, MUMPS, RUBELLA (MMR)
- POLIO ( IPV)
- HEPATITIS B. (Hep.B)
- CHICKEN POX.( varicella)
The last dose of DTP and Polio must be on or
after the child’s fourth birthday. He/She must have 2 MMRs after
the first birthday, a set of 3 Hepatitis B shots, and chickenpox
for children who have not had the disease.
All students entering Kindergarten
through the 5th grade in Kansas schools for the 2008-2009 school
year are required to have three doses of Hepatitis B and one dose
of Varicella vaccine (chickenpox) before starting classes in
the fall.
The 2008-2009 vaccine requirements for
school entry include both 4th and 5th graders. This is new
and these students may or may not have received these vaccinations,
please check your records.
*Please note that two doses of
Varicella (chickenpox) are recommended for children in all
age groups.
PHYSICALS:
Kindergarten and all new students to Kansas schools must have a
physical examination by a physician within the last 6 months.
Please make your doctor’s appointment early.
BIRTH CERTIFICATES:
Bring your child’s official State Birth Certificate. This is
mandatory prior to new student entry. Hospital certificates are not
acceptable.
